By ticking that box you create a screen which can capture the Account Designation details, either revocable or irrevocable- both options have descriptive text to assist you:

When the person who has a Designated Account turns 18, they must open their own account.
A new Investor record should be created for the 18 year old, and a Freshdesk ticket raised, requesting that the Account in question is transferred to the New Investor Record.
